Dr. Michael Carnahan, MD

Dr. Michael Carnahan was born in Lawton and went to undergrad at OU. He attended OU School of Medicine from 1982 to 86. His residency in family practice was done at St Joseph’s program in Wichita, KS and completed in 1989 and is still currently board certified in family medicine. He has been in practice with Dr. McCulloh in Ardmore since that time. Initially, Dr. Carnahan delivered babies for the first few years in practice (which he loved). He has worked in occupational medicine at Michelin, in-patient behavioral medicine, in- patient medicine, pediatrics, and rehab. He has been medical director of nursing homes, home health, and hospice organization, and has also been a teaching preceptor for the OU and OCU PA programs and have taught multiple students through the years. He lled several teams to Uganda Africa to do medical mission work. He and Dr. McCulloh have always had a heart to serve our community and helped start the Good Shepherd Clinic along with others in our community to help the underserved. He and his beautiful wife Shawna and have raised three wonderful children here in Ardmore and are active in the Methodist Church. Dr. Carnahan says that he feels so blessed to serve in this community and to continue to be a part of Good Shepherd Community Clinic.


Kerri Farmer, APRN-CNP

Kerri moved to the Ardmore community in 2007, after being raised in the Ada area. She received both her Bachelor of Science in Nursing and her Master of Science in Nursing from the University of Oklahoma College of Nursing in Oklahoma City. She is board certified as a family nurse practitioner and an active member of the American Academy of Nurse Practitioners (AANP). Her areas of interest include chronic disease, diabetes management, obesity, health and wellness promotion and preventive medicine. As a provider, she seeks to emphasize patient education, patient empowerment, and compassionate care. Her belief is that if allowing the patient to control their health care goals and have input into their own health, they will be empowered to make healthier decisions. She is certified to care for patients from birth to geriatrics. 

In her free time, she enjoys spending time with her husband, Eric and their two children. Her hobbies include gardening, photography and crafting. 


Gina Hernandez-Roberts, PA-C

A long time Ardmore, OK resident, Gina was born in Tegucigalpa, Honduras in 1979. Her family migrated to United States, specifically to Ardmore, in 1982. Gina graduated from Plainview High school in 1998 and then attended East Central University in Ada, OK. She graduated in 2002 with a Bachelor’s degree in Biology. That same year, she accepted an offer to OU’s Physician Assistant Program in Oklahoma City, OK. During her practical year in the program, she married Jeff Roberts from Blair, OK. Upon graduating with a masters’ in Health Science, Gina accepted a position as a Physician Assistant with the Family Health Center in Ardmore.

Gina believes that you should treat the patient and not just the diagnosis. She has over 15 years of experience in family medicine. Gina chose family medicine because she feels like it is the cornerstone of medicine, health education, and preventative care. Being from a Hispanic back ground, she is fluent in Spanish and feels honored to be able to use it daily. Now that she has joined the Good Shepherd Community Clinic, she is excited to continue to offer quality health care.

Gina and her husband Jeff have always loved to serve. She not only does this in her family practice but also by doing mission work. She and her husband have traveled to Uganda, Mexico, and Honduras on several mission trips. After the birth of her daughter Harper, she continued to serve in a different capacity. Gina served on the YW8 board from 2012-2014. She is currently a member of the Board of Trustees for Oak Hall Episcopal School.

On her down time, Gina enjoys spending time with family, traveling, reading, and exploring new cultures and food


Dr. JaHannah Jamelarin, MD

Dr. JaHannah Jamelarin is a highly qualified physician specializing in psychiatry with a vast array of academic and professional experience. She is currently pursuing a Master of Business Administration from Oklahoma City University while holding a Graduate Certificate in Healthcare Practice Management from the same institution. She obtained her Doctor of Medicine degree from Wright State University's Boonshoft School of Medicine and completed her General Psychiatry Residency at the University of Cincinnati's Department of Psychiatry and Behavioral Neuroscience. In September 2013, she became Board Certified in Psychiatry by the American Board of Psychiatry and Neurology.

Dr. JaHannah Jamelarin currently serves as the Chief Integrated Health Officer for the Good Shepherd Community Clinic.  In this role, she oversees the integration of behavioral health and other services to optimize patient care and outcomes. Previously, she has worked in the telemedicine sector providing psychiatric care in multiple states as well as working at institutions such as the Laureate Psychiatric Clinic and Hospital, Mental Health Services of Southern Oklahoma (now Lighthouse Behavioral Health), and Mercy Hospital-Ardmore.  Additionally, she has held academic positions, including Assistant Professor of Psychiatry-Clinical at The Ohio State University.

Dr. JaHannah Jamelarin's service and leadership experience includes serving on the board of Girls on the Run of Southern Oklahoma and serving as a Trustee of Oak Hall Episcopal School. She is also a co-developer and speaker for Teaching with Hope, a program that focuses on mental well-being and coping with stress during the COVID-19 pandemic. Additionally, Dr. JaHannah Jamelarin has participated in several national presentations and is an active member of various professional associations, including the American Psychiatric Association, the Oklahoma Psychiatric Physicians Association, the Oklahoma State Medical Association, and the Ohio Psychiatric Physicians Association.


Daphnee Jones, PA-C

Daphnee Jones was born in Arkansas and moved to Stratford, Oklahoma in 2015 where she graduated from in 2016. She graduated with a Bachelor of Science in Biology in 2020 from the East Central University. She then attended The Oklahoma City University Physician Assistant program and graduated in 2023. During her time in PA school she served as the class secretary for 28 months. She rotated at Good Shepherd Clinic two different times as a student before joining the practice in June of 2023. She and her husband Ethan have been married since 2019 and have a farm with a variety of animals in Roff, Oklahoma. She became interested in family medicine after her first rotation at GSCC after getting to experience the "whole patient" approach. She always wants to do what is best for each patient and she feels like she can accomplish that here at GSCC.

Daphnee enjoys spending time with family/friends, tending to her animals, and reading a good book.


Michelle Martin, APRN-CNP

Michelle Martin is a Family Nurse Practitioner.  Michelle is a native of Skiatook and currently lives in Wynnewood.  She earned a Bachelor’s of Science in Nursing from Oklahoma City University in 2006.  She worked 11 years as an RN in pediatric hematology/oncology and public health.  She earned her Master’s of Science in Nursing in 2017 from the University of Oklahoma.  Michelle had been practicing in Pauls Valley until joining Good Shepherd this year.  She has been married since 2006 and has two children, Blaine and Carter.  Michelle is active in Kappa Phi, a Christian women’s organization, and with her church. In her free time, she enjoys reading, cooking, watching OSU football, and spending time with family. Michelle loves taking care of several generations in a family and is looking forward to caring for families in the Ardmore area.


Dr. Thomas McCulloh, DO

Dr. Thomas W. McCulloh, an Ardmore Oklahoma native, graduated from The University of Oklahoma in 1972 and earned his medical degree from Oklahoma State University College of Osteopathic Medicine in 1977. He completed his Internship at Garden City Hospital in Garden City, Michigan and completed his residency in 1983 at Wayne State University in Michigan. Dr. McCulloh founded the Good Shepherd Community Clinic in 1996 which has grown from a small volunteer clinic treating just 85 patients to a full time innovative medical facility caring for more than 9,000. He is the Director of Skilled and Long term care units at Elmbrook and Southbrook Nursing facilities, Rehab Physician of Mercy Rehabilitation Center since 2010 as well as Adjunct Clinical Professor of Family Medicine Oklahoma State University College of Osteopathic medicine. He is Board Certified in Family Practice with added qualifications in Geriatric Medicine by the American Osteopathic Board of Family Physicians and was awarded Doctor of the Year in 2008 by the Oklahoma Osteopathic Association.

Dr. McCulloh believes in the importance of working as a team with patients and families to provide the best health care possible. In his spare time, he enjoys doing home improvements and spending time with his family and dog Millie.


Zann Shelton, APRN-CNP

Zann has been providing excellent women’s healthcare in Southern Oklahoma for over 32 years. She began her nursing career in 1990 after receiving a Bachelor of Science in Nursing degree from East Central University. As an RN for 17 years, she gained vast experience in pediatrics, women’s health, and maternity care. In 2006 she graduated from Texas Woman’s University with a Master of Nursing-Family Nurse Practitioner Degree. Zann is a board certified Family Nurse Practitioner through the American Association of Nurse Practitioners (AANP). She also received a board certification as a Lactation Consultant in 1999 and has over 25 years of experience helping many moms and babies achieve a successful breastfeeding experience.

Zann has been happily married to her husband Bobby for 32 years. They have two married children. She loves the summertime and spends it with family and friends in the outdoors.  Zann’s passion is to provide personalized quality health care to women from all backgrounds. She strives to create a relaxed office setting so that both women and teens can feel comfortable discussing  any needs or concerns.

Dr. Terrick Washington, DDS

Dr. Terrick Washington is a native of Ardmore, Oklahoma. He graduated from Plainview High School in 2009. He obtained a Bachelor of Musical Arts degree from the University of Oklahoma in 2014. Dr. Washington is a graduate of the OU Health Sciences Center College of Dentistry, where he received high quality training and experience in preventive and restorative dentistry. He, his wife, and his three children love the Ardmore community.


Lacy Westfall, APRN-CNP, WHNP-BC

Lacy was born in Ardmore, OK in 1987. She is a long-time resident of Love County where she resides with her husband Jeremy and their two daughters, Landry & Joley. She attended Murray State College School of Nursing, graduating in 2008. After 5 years of nursing, she returned to school and completed her Bachelor of Science in Nursing from Oklahoma City University Kramer School of Nursing in 2013. During this time Lacy gained experience in various aspects of women’s health care including Labor & Delivery, Postpartum couplet care, Women’s surgery, OBGYN, urogynecology, lactation support and education, pelvic floor therapy and public health. Her experience in nursing as well as her own personal medical experiences led her to pursue a Women’s Health Nurse Practitioner degree from Frontier Nursing University. Her goal is to increase access to care for the women in our community. Lacy believes all women have the right to holistic healthcare in a welcoming and nonjudgmental environment.  She was drawn to GSCC and its mission to serve the community and is excited to share their core values with the women she cares for.


Heidi Wright, PharmD

Heidi was born in Ardmore, OK in 1981, but grew up in Davis, OK her whole life.  She is a 1999 Davis High School graduate.  She went on to Oklahoma State University for her undergraduate coursework.  In 2002, she was accepted to the University of Oklahoma College of Pharmacy and completed her Doctor of Pharmacy degree in 2006.  She then moved back to Davis where she worked for her uncle, Randy Moore, at City Drug for the next three years.  On January 1, 2010, she purchased City Drug and ran it successfully for the next 11 ½ years before selling her business in August 2021.  During that time she served on the OU College of Pharmacy Alumni Association, serving as President during 2013-2014.  She is also a preceptor/adjunct faculty through the OU College of Pharmacy where she has mentored and taught several students in her independent retail pharmacy setting.  She and her husband, Jason, and their two boys have made their home in Davis.  They are members of Midway Hill Baptist Church where Heidi teaches a children’s Sunday School class. In her spare time, she loves to watch her boys play baseball, football and show livestock.


Jerry Wright, DPh

Jerry grew up in Hollis, Ok and attended Southwestern Oklahoma State University where he obtained a B.S. in Pharmacy degree. He has 43 years experience in the pharmacy field. Jerry is active in his church and has gone on several rewarding medical mission trips to Nicaragua. Jerry enjoys taking care of his customers and feels like he is making a difference. His hobbies include fishing, traveling, and following OU football.
